Are Knock Knees Genetic? Understanding Hereditary Genu Valgum

A practical guide to the causes, warning signs, and treatment of genu valgum — from bracing to corrective surgery — for families in Yavatmal and Central India.

What Are Knock Knees (Genu Valgum)?

Knock knees, medically termed genu valgum, is a condition where the knees angle inward and touch while the ankles remain apart when a person stands with legs straight. Mild degrees of this alignment are extremely common in toddlers between ages 2 and 5 and usually correct on their own as the child grows. However, when the inward angulation persists beyond age 7-8, worsens over time, or is markedly asymmetric between the two legs, it warrants a proper orthopaedic evaluation. Is Genu Valgum Genetic? Understanding Hereditary Causes

The honest answer is: it depends on the type. Hereditary or developmental genu valgum, on the other hand, can run in families and is often linked to:

Parents with a history of knock knees, particularly if it persisted into their own teenage years, should have their children screened proactively rather than waiting for symptoms to appear.

Tip: Measure the gap between the ankles while your child stands with knees touching — a gap greater than 8-10 cm after age 7, or one that is increasing over 6 months, should be assessed by an orthopaedic specialist rather than monitored at home.

Symptoms to Watch For

Beyond the visible inward angling of the knees, look out for an awkward or waddling gait, frequent tripping, knee or ankle pain after activity, unequal shoe wear on the inner sole edge, and visible asymmetry between the two legs. In adolescents, unaddressed genu valgum can contribute to anterior knee pain, patellar instability, and — over years — accelerated wear on the outer compartment of the knee joint, a pattern that can predispose to early osteoarthritis in adulthood if left uncorrected.

How Genu Valgum Is Diagnosed

Diagnosis starts with a careful clinical examination — measuring the intercondylar and intermalleolar distances, checking gait, and assessing limb length. Standing full-length X-rays of both legs (hip-to-ankle) are the gold standard, as they let us measure the mechanical axis deviation precisely and identify exactly which growth plate is contributing to the deformity.
